Please consider making a monthly donation to support our ongoing efforts in delivering top-quality care to the elderly. You can easily indicate your preference for monthly processing by checking the box below the donation amount and filling out the form. Once submitted, your monthly gift will be automatically processed using the credit card information you've provided.

Is my contribution deductible?
Donations to the Cynthia F. Blaizes Foundation may qualify for tax deductions as allowed by law. Please consult your tax advisor to confirm the deductibility of your contribution.

When is the initial charge to my credit card scheduled?
Your initial donation will be charged immediately upon online creation. Following that, subsequent donations will be charged monthly on the same or nearest business date to your initial sustaining donation. Sustaining gift requests made via mail or phone will be processed within 24 to 48 business hours upon receipt.

Can I expect documentation of my donations for tax purposes? Following each monthly donation processing, you'll receive an email containing your donation receipt as an attachment. It's advisable to print or save these emails and attachments as proof of your contribution. If you don’t find these notifications in your Inbox, kindly check your Spam or Junk folders.

For phone or mailed donations amounting to $10 or more, you'll receive an email with your donation receipt attached. Please keep this email as documentation of your gift.

What is the process for canceling or modifying my recurring donation? You have full control over your donations. To adjust your monthly giving amount, simply send an email or give us a call. You can reach our Donor Relations & Services team at info@cfbfoundation.org or by dialing our toll-free number 1-888 CFB 3089 (1-888-232-3089) and selecting option #2. Our Donor Relations & Services department operates Monday to Friday, from 9:00 a.m. to 5:00 p.m. PST, excluding holidays.

Cancellation requests received before the monthly charge will be processed in the same month they are received.
